Welcome Aboard!
- New patients are always welcome at Alta Loma Pediatrics. We are excited to meet you in your first visit , please arrive 20 minutes before your first appointment to fill up paperwork if needed, you can also download and print our forms here, to fill out and bring with you at your next appointment
Bring Company
- We require that one of the parents be present on the child’s first visit, unless the child is under the guardianship of another adult, then the legal guardian is required to be present. On the following visits your child may be accompanied by another caregiver that you designate, after signing the “Assignment of Caregiver” form
Always Come Prepared
- It is a good idea for parents to bring the child’s medical records to at least the first visit if possible, including copies of vaccine records, Lab results, imaging studies, specialist notes, medications list, and any other pertinent records, as well as names of pediatricians previously involved in your child’s care so we can keep your child’s record updated
